    This entire clause is referring to "the amendments" (plural). That’s clear from: "which have sparked..." (plural verb) But then: "and has polarized..."  (singular verb) If "the amendments" have sparked debates, then they also polarized opinions — same subject, still plural. So, the verb "has" in part (d) should be "have".
